Anyone who wishes to be a head or assistant coach must complete this form or have completed it in prior seasons.
Return or mail the completed Authorization For A Background Check Form, to Tallowood Basketball, Mrs. Betsy Samu, 555 Tallowood Drive, Houston, TX 77024 as soon as possible.
